Change to Orthodontia Medical Necessity Criteria

Effective January 1, 2026, the Salzmann Evaluation Index will be replaced by the Utah Medicaid Index of Orthodontic Treatment Need (IOTN) to determine medical necessity for orthodontic treatment. Please read our provider bulletin, which includes specific guidance on when to use the new IOTN form, as well as other important information about this change.
